Knowledge Base Content Survey plugin - only works with Documentation Theme (not Refined Wiki)

I use Refined Wiki's Original Theme. But it seems the Survey Plugin refuses to work with it:

I note the source code for the plugin is available at https://studio.plugins.atlassian.com/source/browse/CKBSP - has anyone built out a bridge to make it work?

Same issue, but I can't get the plugin to work even if I switch to the Documentation theme. The space admin entries show up, but nothing changes on the content pages (tried various combinations of inserting {kbsearch} and {kbsurvey} in the customization areas). Confluence 5.1.3.
