Interactive page which takes input and outputs a pre-made script

So there would be an option on my Confluence page which prompts the user to give a value, on doing so the page would then return this value as part of a script.

i.e. Input Value: Fred Flintstone

Output onto page: insert into names values ('Fred Flintstone', now(),'yes');

 

Going one step further I'd like to have a drop downs of options to choose from which look at different scripts and have the option to request for  multiple parameters depending on the script.

Can this be done via the macros or has this been done before?
