Is there a way to import from a on-premise Jira implementation that has been customized to have the same fields, schemas, workflows, etc.. into On-Demand version of Jira?

Yes, you would need to take an xml backup and import it on the on demand version (via webDav). Please note that the xml import will wipe off any existing data on the on demand version.
