How do I add a disk to my server without rebooting?

Given a new disk volume (VMware, AWS, Xen, etc) added to a virtual machine running Linux, how do I rescan the SCSI host to add the new volume to the Linux server?

(NOTE: I am adding this because I get asked this a lot in IRC and other places.)

Here's the one-liner I use a lot:

for i in $(ls /sys/class/scsi_host/); do echo $i; echo "- - -" > /sys/class/scsi_host/$i/scan; done

Execute this from a terminal after the disk is added and Linux will rescan the SCSI bus to find the new disk.
