How can I create an issue from a rule in Insights Automation?

We have a very simple rule setup to send an email and create an issue, but our issue isn't being created.  We are not sure what is preventing this piece.

Before anything else, check permissions. Validate whether the user you are using as the reporter of the new issue actually has create issue permission in the project you want the issues to be created.

And next, check whether the configuration of your project allows for issues to be create based on what you want to include in the issue (mainly check issue types and custom fields):
