Error on trying to create a survey using Adaptavist Survey Plugin

I've installed adaptavist Survey plugin and when trying to create a survey I get a System error:

Thanks for the help. The problem was in incompatibility of confluence and plugin version.

Firstly, ensure that you have installed a compatible Survey Plugin version with your Confluence version. You may check that from the plugn's page: https://marketplace.atlassian.com/plugins/com.adaptavist.confluence.survey. If that is not the case, you may directly contact the author of the plugin at :
