Hello Aharon,
Here are the steps to install and configure the plugin:
1. Install it.
2. Go to the Custom fields page and add custom 'Text Field (multi-line)' field name: "Checklists"
3. Go to the View Field Configurations page and associate the field Checklists to the appropriate screens.
Then Checklist field should be shown in the Create issue popup. Please follow the following format of a line to add checklist items:

- to be checked item

+ done item
