intergrate and external page / form

I am trying to integrate and external page / form into the JIRA Service desk customer portal.

We have a need to included some external system data with our CUSTOMER Portal in JIRA service desk. We are not sure the right way to do this...

basically we have two forms one a very detailed page what contains data that can not easily be integrated into the fields in service desk. We need to pass data between these forms in someway shape or form. 

Our use case wish is

1. customer logs into service desk portal

2. customer is taken to external form pass ID case # to external form

3 . External FORM

4. DATA passed back to customer portal... and submited..

Any idea the best way to do this, we are new to JIRA service desk and are not sure the best approach to accomplish this requirement...
