Thanks for pointing that out, as it turns out there was a JavaScript library update in Bitbucket Server that broke the plugin's front-end. I've just published a new version (3.0.5) of the plugin on Marketplace that is compatible with Bitbucket Server 5.5.0 and up.
You should see it available as an update in the plugin manager in Bitbucket, or you can download the latest version from Marketplace.
