Hello Jason,
1. After you installed the plugin you need to go to the Custom fields page and add custom 'Text Field (multi-line)' field name: "Checklists"
2. Then go to the View Field Configurations page. There should be Checklist field shown. Associate the field Checklists to the appropriate screens.
3. Then Checklist field should be shown in the Create issue popup. Please follow the following format of a line to add checklist items:
# checklist items group as headline
- to be checked item 1
+ done item 2
