The default modules have changed as of 3.0.
Go to Administration > Add-ons > Manage add-ons > search for JIRA Gerrit Plugin. From there on the right side you should see "N of 28 modules enabled" (on my installation it's 25 of 28). If you click the plus sign next to that, you should get a list of all the modules enabled on the plugin.
A normal install should have 3 modules disabled by default: "Gerrit Reviews Tab Panel" (this is the a tab that appears next to Comments/History/etc), "Subtask Gerrit Reviews Tab Panel" (same as tab panel, but for the issue's subtasks), and "Gerrit Reviews Side Panel" (appears on the right side, under People and Dates).
The module that is enabled by default now is the "Gerrit Reviews Web Panel", which appears on the issue page, above comments and below Sub Tasks or Issue Links.
Can you confirm which of those 4 modules is enabled? And can you try toggling them to see if any others show up?
