Forms for Confluence giving Oops -error

Forms for Confluence 5.0 gives Oops -error when trying to submit a form. I have uninstalled / reinstalled the plugin and tried very simple forms. Is there a known issue? Confluence version is 5.8.10.

Hope this finds you well. We have released Forms for Confluence v.5.0 which has new exciting features. Firstly, please let us know if you have been using an older version of Form Mail plugin prior to the installation of Forms for Confluence v.5.0. If yes, please let us know the version of Form Mail you were previously on. With the release of Forms for Confluence v.5.0, we have applied many changes to the plugin, so please read our Release Notes and also the documentation for Upgrade path : http://www.adaptavist.com/doco/display/F4C/Release+Notes+5.0

You will not receive any errors if all the macros are migrated successfully.
