Form Mail NG not sending emails


I have configured Form Mail NG and created a form. However, when I submit the form, i see no trace of the email going out. I don't see any information in the Confluence logs, its not hitting the maillog, and I opened up a network trace and nothing going through the network on port 25 (smtp) either.

Does anyone have an idea of what I might be missing?
