Creating issues in Another Project from Workflow without modifying current issue

Hi,

Got a scenario in project X, where I want to trigger the creation of an issue in project Y. 

I found a couple of examples ,BUT that didnt just trigger the creation screen in the project Y. 

Detailed workflow idea:
1. Create issue

2. Start issue

3. Move through transition by hitting button Z.

4. issue in Project X gets closed

5. the create new issue screen of Project Y appears

6. after the issue is created the old issue from Project X is linked to the new issue of Project Y

As today Im not able to do that simple task...

Tried:

"Bob Swift Atlassian Add-ons - Create on Transition"

But that add-on does not let you just trigger New Issue screen... 

I hope it was understandable....

A Script will do it because with Bob Swift Add-on you will have to have both issues created and once first one is closed it triggers the second one to get started.

Remember that are fields that need to be filled out.
