Confluence Easy Forms - created a report page for an Easy Form on another page but page not found

I've successfully added an Easy Form to a page in Confluence but when I define an Easy Form Report on another page I get an error 'Not found' when entering the URL for the page with the form.

I've double checked the definitions which are correct.  Could this be a permissions issue? Or an special syntax requirement?
