Approve by Email Fails: Email This Issue add-on Please help

Hello,

 

This plugin is failing to process incoming emails. We have followed the tutorial to include an Approve and Decline button in the emailed notifications and below is the following Log Details:

I have confirmed multiple times that the mail handler is looking for the correct transition. Why when I test this procedure it works but when one of our Service Desk customers sends an approval email we receive the above error?
