Agile Story Map Evaluation Issues

We are running JIRA v6.3.14 and have installed Agile Story Map Plugin v2.5.0.  

The license details are:Evaluation, Unlimited-user commercial license, Standard, expires 11/Mar/15

License status: Valid

We have left all the defaults for the configuration.  When we select Agile->Story Maps, we get the Agile Story Map page; however, we only see a header with the Hide/Show Favorites button, Manage Maps, Add Epic, Create Epic, etc. buttons.  There is a table of 2 columns below that, but it is completely empty.  

We are evaluating this as an enterprise component for our company (2000 user license).  Our 30 days is ticking away, and we are not able to do any evaluation.  Please help.

Hi Brian, can you please add a screenshot of what you are seeing. Configuration should not be necessary - default values are sufficient.

OK there is something wrong! please press F12 to open the browsers debug console, change to 'Console' tab and press F5 to refresh the page. Then please send me this log (if you want per email

I submitted an identical question last week. We moved to a new instance (better performance) over the weekend and post migration the page now loads after 3-4 minutes. Unfortunately all issues are showing as "Unlinked". I have added "Epic Link" to the add-on config and have verified that the field is complete for issues. 

There is most certainly something wrong with this plugin.
