What is the best template to use for creating an Environment Booking project?

I am new to Jira and need some help in setting up a Booking request form for Environments. I want the form to have the ability to show contentions at the time of booking and also want to have a visual representation of those bookings. I do have Golive which has an Environment field and Timeline.
