Searching form to list (remember data)

Hi everyone,

I am searching for a plugin or way to manage our guestlist. 

So if a costumer is coming to us. We have to fill out several information like name, address, company, number plate. We want to have a form that we can fill out. And this form will put the entries to a list. Then we want to have a check in and out button or field. 

Has someone an idea?
