I am using the Scroll PDF Exporter to export my confluence pages as PDF. Therefor I want to include the associated version history page, I created for the actual page.
This version history is a child page, but not the only child page of the actual page. So I want to include this page with the include macro as a static page right after the title page of my PDF template.
Is there a way to use this macro within the template designer?

Yes, you can use the Scroll PDF Exporter placeholders in your static pages.
Simply create a new static page and insert the placeholder
You can then change the position of this static page via drag&drop and on export the content of page with pagetitle "include" (as defined in the example above) will be rendered in the exported PDF file.
