It appears that the only way to create a form in Proforma is via the manage apps page which is only accessible by an admin. Is it possible for a regular user to create a form?

I am pretty sure that you must be a Jira Admin or Project Admin to be able to create ProForma Forms.

Yes that's correct (although we'll be migrating to cloud later this year).

If you`re searching for an alternative solution check out the Smart Jira Forms add-on, developed by my team. Here you can create a custom form with a drag-and-drop menu as a regular user.
