Oauth token was globally revoked. Wish to know the reason.

We have registered an oauth app for auth with Atlassian. On refreshing user’s token (using https://api.atlassian.com/ex/jira/{customer-orgId} endpoint), we received the error: "unauthorized client", with description "Token was globally revoked". I want to understand what is the reason behind this message (it’s the first time we’ve seen for some customer), and on verifying with customer they mentioned there were no revocations from their end.

P.S. I could not find this in any documentation, please point it out if I missed it.
