If you are talking about this app, then no. It is only availabe on Cloud.
If you want to see the hosting options of an app, you can click on the menu on the right side of the listing:
If no menu opens, like with the linked app, then that is the only available hosting option.

No idea. To find this out you will need to contact the vendor directly and ask them.
However, since there isn't even a Server version you'll have to wait until that comes out first. (Data Center apps are basically just Server apps that went through an approval process, which can take a few months)

At the minute the App only works on cloud as Sven said.
But Jira server and Data center are in our scope for 2020.
We'll have more info by the end of this year (not so much to wait though)

Without going to deep into AWS code pipeline, sending tests to C4J is done by a curl command you can add to your CI process. I guess it could be done in AWS code pipeline as well.
