How to create Intake Form without custom Fields

We would like to capture project intake request, from requester, through multiple questions on a Form.

We would like most of the inputs to be captured under Description, in Jira Ticket/ issue.

Also, during the lifecycle of the Intake request, additional inputs will be updated, under description, by various teams involved, to finally get a complete project charter. 

Is there a way to achieve this using Dynamic forms?

Hi Anna - Welcome to the Atlassian Community!

You can probably do most if not all of this with a Jira Work Management project. You can create a simple form very quickly. If you need to conditional fields in the form, then you would need to add an Advanced Form to your instance. Contact Atlassian support to have them turn on that functionality for your instance if you don't have that already in your project settings for JWM projects.
