How To Get a Specific Form to Show In A JSM Request

Hello!

I have created both a form and an automation for a specific task to run but am trying to see how the form that I've created will show in the automated service "task/ticket" within the JSM portal. I see that I can add the form once the "task/ticket" is created (and I can confirm the automation is set correctly) but I am having trouble trying to figure out how to set it so that only the form I created shows within the ticket and without me having to search and add it that way. Is it possible? If so, could someone tell me how I'd go about doing this? The form is not external facing.

Hi @LaKisha Brown 

Are you looking to replace the standard request form in the JSM Portal, with your custom Form? So a customer would fill that in instead?

If yes...

  1. On your JSM Project, go to Project Settings > Forms
  2. Access the form, and select the Settings tab
  3. Toggle the Request Form under Locations and Request Types to ON
  4. Choose the request type(s) to replace with this form
  5. Save changes

  • What is the process you'd like to implement, step-by-step?
  • How the Automation rule relates to the form?
