I am from the 'Azure Pipelines for Jira' dev team and the installation of the app (which was working last week) is failing now even though we have not made any changes. Customers are not able to install the app and have been raising multiple support requests.
When clicking on 'Get It Now' for installing Azure Pipelines for Jira I get the following error:

In our system it looks like the add-on was correctly added to your instance and the evaluation started on the 5th of April (same day you created this thread).
Can you kindly let us know if you actually managed to install the app or if you are still having issues?
Also, in case you still face the issue, can you confirm everything you do is going to "Find New Apps", you search for the app and you try to add it, or if you are doing anything else?

Thank you for the response!
I verified just now that the app has not been installed on 'benten420.atlassian.com' - it is not showing up in my list of apps, and I tried to install it again and got the same error.
We also tried in some other accuonts where we hadinstalled the app last week and then uninstalled, got the same error there as well.
It might be that it is installed but not showing up due to some issue?
All I am doing is -going to "Find New Apps" > search for the app > try to add it (Get It Now)

I have double-checked and I can see that indeed the App Azure Pipelines for Jira is not installed on your Jira free instance.
The apps that are correctly installed on your site are TFS4JIRA Azure DevOps integration and Git Integration for Jira.
Once this has been said, I am able to reproduce the issue on all my instances. Free or commercial license doesn't matter, you always get the same error and in Jira logs I can see:
Error contacting remote application 500:[Internal Server Error]:{"$id":"1","innerException":null,"message":"TF400898: An Internal Error Occurred. Activity Id: 9fde909a-c715-4ffa-a171-1f334a1637a7.","typeName":"Microsoft.Azure.KeyVault.Models.KeyVaultErrorException, Microsoft.Azure.KeyVault","typeKey":"KeyVaultErrorException","errorCode":0,"eventId":0}
As you can read the error is returned by the remote application and, actually, TF400898 is a general error which could wrap almost any kind of server-side TFS error.
Therefore, you may want to get in touch with the vendor using the resources listed in the support tab of the marketplace page for the app:
