Create sub-task only when custom field is checked

I have a scenario where when I create a ticket in any project, that it creates a sub-task only when I have a custom field checkbox checked. I know I can use the conditional execution, but I do not know how to write it the custom field name is "Impact to Data Team" and it needs to be created when "CRM" or "Data Warehouse" is checked. Any help would be appreciated
