Create a form in confluence and save it as a PDF on a space in confluence

Looking for a solution.

I would like to create a form.  It's a repair form that is fill able, can attach a picture  and can be saved on confluence for historical purposes.

I'm a beginner user to confluence so anything easy to start off would be great
