Automation on transitioned issue not firing (but it does other times)

is there any reason why it works and why it doesn't? I have open, then when it moves to assigned its supposed to fire an automation. I can bring the status back to open and then push it to assigned and it will work fine. not doing anything different. I check the audit log and it show nothing for the first time when it doesn't work.
