I've previously written an article here which explains sending a list of issues to Slack on a schedule: Morning Slack digest of Open Jira Issues
Much of the same rule setup will apply. You may want to send the list to a Slack channel instead of individual users - and you mentioned wanting to send the report weekly instead of daily.
You'll also want to tweak the initial JQL to capture issues that have breached whichever SLA(s) you are monitoring. This article describes how to filter for SLAs . The JQL could be as simple as
"Time to first response" = breached()
