one JIRA issue in a project has no workflow options

Here's something we've not see before:  in two different projects we have one and only one issue that has no workflow transition options.  For anyone.  Regardless of user permissions.  Both projects have a couple thousand issues.

Couple months ago we upgraded to 7.1.  Not sure if we've seen this in earlier versions.

Any ideas?

Try the integrity checker.  It should find and name your issue and offer to fix it.

JIRA keeps workflow information in two places, and sometimes they can drift apart (large updates and errors can cause it, but the usual culprit is broken add-ons or scripts).  When that happens, JIRA doesn't know what state the issue is really in, so it can't display the transitions.

Are there log entries we can look for that would point us to which add-on or script is broken?

I'd look for the words error, warning, or fatal in the same area as the word "index" (or indexing)
