how to include headings for cascading lists

I just want to include heading for a simple cascading list.  What am I missing?

Sorry.  I guess I need to add  more info...

I am adding a Custom Field and it is a Select List(cascading).  My management want me to add heading by each of the 2 selection fields to better identify what kind of data is being requested. 

IE.  the first field is for document selection and the 2nd field is to pick what version/revision of the document.

If I need to supply more information please let me know.

I think you're missing some context.  Include it where?  What do you mean by heading?
