[addon-com.intenso.jira.plugin.multilevel-group] UI not showing properly

Hi,

We have installed Multilevel Group By report version 1.2

Our JIRA version is 6.0.6

It could able to create a report however the view is not proper as attached.

Kindly suggest what can be done to make it proper..

Any support on this is really helpfull

We bought this plugin especially for the view it provides, it would be waste of our money if it could not able to provide proper view.
