We are currently running Jira 4.1.2 and want to upgrade to 4.3.4.
1. What would my upgrade path be?

You should follow the 4.3 upgrade guide - at a minimum, and also be running through this upgrade on a "staging system" before doing it on your production - this is not really an optional step imho.
Recommend reading and engulfing release notes of every release along the path before beginning - so you have a general familiarity for all the feature changes and bug fixes. Do not assume anything.
99% of the trick on successful / smooth upgrades is doing your homework and not making assumptions.

Also its a good idea to let a pilot group of users evaluate, digest, and communicate the many changes to your user community proactively vs. just shocking your users. There's alot of new features between those releases.
(Why are you only going to 4.3.4? Why not go to the latest 4.4.1 - especially since you will have to test and evaluate on staging anyways?)

If you move to LDAP users this step is a little fraught. Ensure that you have a administrator defined in the local database without a corresponding ldap user. You will need to use this account to manage the ldap configuration.
There are some unexpected interations between your ldap and internal jira users. Ensure that your ldap filters work as planned as missing ldap users can be removed from internal groups.
