Survey for Jira Software

Hello Everyone,

We are managing the IT projects on Jira software. When we complete the project (when status is done) i want to send and survey to stakeholders. Stakeholders are in the 3 different custom fields (User Picker) which are sponsor, Product owner and Project Manager. 

I need in the 5 question

Please help me if there is any add on.
