Something went wrong appears when try to create a ticket with issue collector

Hi, we have created a issue collector form in our google site, this is to open a ticket when a user need add people to a project in jira, etc. we have a project created and when we tested all was good but when we released de page to all the users, they have this error, please your help:
We have already erased the issue collector and created another one and the problem still the same.
