Can you please clarify what it is you require? Are you asking for more than one Customer Portal in Service Desk? By different URL do you mean the <your-instance>.atlassian.net part, or something else?
Lastly, explain what you mean by automatically assigning a field in the form. If you can provide a use case or screenshot of an example, it will help us.
