Link issue collectors to html buttons not tabs

We are attempting to make a web page where a tester could submit Bug, Task, Improvement, Epic by clicking on html buttons (not the trigger tab styles that are standard to the issue collector). We are not javascript or jquery power users. Any examples of this?

This widget could not be displayed.
One of my students came up with this solution. You generate Issue collectors for whatever cases you are collecting. You format a web page and embed the javascript as indicated and then link the script to the button.
