Jira Status value is duplicated in transition history

Hi,

We have started to see issues (c40+ a day) where the 'From' and 'To' Status values are the same.

This appears to the end user as though the transition has failed but the issue actually has been updated. To resolve the issue the user has to re-transition it back and forth through the workflow.

Can anyone offer any advice on what is causing this to happen?

No, all transitions are to different Statuses.
