For JIRA On Demand, is there any add on or way to make the system send an Text, automated phone call or any other type of notification other than an email? This would only be sent if the issue was a Blocker.

For onDemand it is not possible. But most easy way to do that - is to set up some triggers on your email box itself - you can make a email filter that forward letter with some text to sms provider, phone call translator etc.
