How to customize the Create Issue screen

For example, I want to add a 'Function Area' field in Create Issue screen, the value including Compensation, Personal Information, Organization, Staffing

create i select list type custom field with those values as described here

and create screen and add the field which you have created and assign it to the create issue operation
