How do you trigger an email sent when a custom field (Approver) is populated

Couple of questions:

How do you trigger a notification email sent to the email value of the field when a custom field (Approver) is populated?

Granting an email notification has been sent to the email value of the custom field, how can a reply to that email be made part of the comments of the original ticket?

This widget could not be displayed.

You can't do it without customization and unfortunately that is not allowed in OnDemand instance.
