How do you reorder fields on the CREATE ISSUE pop-up?

Hello, I have several custom fields I would like to appear in a certain order when users are submitting new backlog items using the CREATE ISSUE feature. Adding your own custom fields to the form is easy but I cannot see an easy way to reorder the fields. Any ideas?

If not - is there a way to predefine some text in the description field so I can prompot users to add the description as a user story. ie, when they create a new issue, they will see a form with a description field that reads:

As a...

I want...

So that...

On the screen definition, you can move the fields up and down.
