We would like to be able to add attachment in a table built in a form. Here is an example of what we would like this to look like.
Here Step # would be a multiline text field, along with columns 2/3/4/6/7; column 5
Test Case Status would be a drop down field. The last column would be a multiline text with the ability to add screenshots.
Is this option available?

Here is how you can do it(but it doesn't look good)
You add a table, then short text inside it so the customers can enter data.
the screenshot section you can only use the attachment field. Neither the paragraph or the long text allows you to paste screenshots. the attachment doesn't allow you to but you can select the screenshot file.
Here is how the AGENT sees the ticket:
The table cannot be widened. That's a JSM limitation on how it displays the form in the screen. It doesn't use the whole screen.
You could create two sections:
That will be a bit better IMHO.
Or you could test the forms plugins from the marketplace. You can test a couple and see if they have what you need (you have a 30days free trial and you can always request an extension to the creator)

JSM has a form you add it to the customer portal.

Do you know any solution for Jira Software?
From what I see, the text field does not allow inserting screenshots, and there are no other types of fields (like attachments) when adding new subfield in FORM:

For Jira Software OOB there isn't. You have several apps that can help you:

But I've found this in the community:
There seems to be a backdoor to enable the forms but with a shared link.
I just tested it and it works! you just need to save the form, assign an issue type and then share the link /jira/forms/create , but the customers need to select the proper project and issue type though. no way around that.
test it and let us know if that works for you!
