How can i create additional field "+add more"

Hello everyone. I need your advice. How can i create additional field, that reveals a 2nd field if the button is pressed? Look at my screenshot please.jira_ladnyi.png

Jira does not have multiple-entry fields like that. 

It doesn't have fields that are compounded like that either.

You would need to write or find an app to provide both of those structures.
