The contact admin form on the login screen for JIRA is vague and I can't find anywhere to customize it.
I'd like to use an issue collector instead. Is this possible? If so how?

You may try setting your issue collector on a public Announcement Banner.
This way people on the login screen should also be able to access it.

While this is a valid option, I'm looking for a way to do this if my announcement banner is private.
