Edit Screen Comment section Heading Edited

It is weird to see this, In edit screen mode on tickets, at the very bottom, we have a default comment section. Somehow I do see two headings for one comment section like the image attached here. How to solve this and display one one comment as a heading? I don't see this comment field added in any of the screens. It appears only when we click edit from the ticket.

This widget could not be displayed.


Can you please confirm if your system administrator has performed any customizations? Behaviour plugin for example?
