Does managing attachments suppose to trigger an Issue Update?

If I add or delete attachments, is that considered updating an issue and anyone that is to receive notifications when an issue is updated, is then sent an email?

Since comments have their own notifications, I thought attachments were not part of this update...but, trying to confirm if the notifications that we did receive were expected results.

This is indeed expected behavior if configured. The issue updated notification is configured in the default notification scheme.
