We do design reviews/preliminary risk analysis on stories that get developed. Currently we are using an xls checklist, with ~20 rows, to capture this information and attaching it to the related stories.
This list includes a number of items that we check the story against (such as documentation needs), specific actions related to this item, and the responsible party.

Is there a plugin that you know will help us with this? We've considered Profields and iGrid but are unsure if either of those will fully address this need.

Not sure it will completely address your needs but you may want to take a look at the Checklist plugin. The checklist may have by default the text in bold and the user can complement the description before checking out the item.
