Does uninstalling CCC Last Comment using JIRA Add-on manager remove all traces of the add-on or does any further action necessary?

Before removing any add-on that adds customfield types, you should remove customfields of that type manually (via administration), otherwise the information about them will remain in the database.

I suggest you reinstall it, delete customfields and remove addon again.
